description A number of laboratory indexes has been proposed as potential marker for prognostic factors of stroke, one of them is D-dimer value. D-dimer may be useful to determine the extent of fibrin development as well as to assess wether there is a change in the process.The objective of this study is to assess the role of high D-dimer value for the prognosis of infarction stroke. This is a prognostic study with prospective cohort design of patients treated in Stroke Unit. Quantification of D-dimer value was measured with enzyme linked fluorescent assay (ELFA), meanwhile infarction stroke outcome was determined by the difference of SSGM score between admission and day 5 post-stroke. Relative Risk (RR) of higher D-dimer values compared with normal D-dimer value in infarction stroke patients was calculated, with p value of < 0.05 is considered as significant. Total 62 patients with infarction stroke was enrolled in the study, 40 of them had Ddimer value > 500 ng/mL. 28 patients had poor clinical outcome. Univariate analysis results indicated that higher D-dimer value (RR = 2,530, p = 0,015), admission SSGM score (RR = 1,957, p = 0,019)), diabetes mellitus history (RR= 1,855, p = 0,044) and dyslipidemia (RR= 2,455, p = 0,026). After multivariate analysis, D-dimer (RR = 5,892, p = 0,008) and dyslipidemia (RR = 4,351, p = 0,041) were independent prognostic factors for clinical outcome in infarction stroke patients. Higher D-dimer level is independently associated with clinical outcome of acute infarction stroke patients. Patiens with high D-dimer value had RR of 5,892 (95% CI 1,585-20,836) for worse clinical outcome with considering the limitation. Key word : D-dimer, prognostic, infarction stroke
