Summary:Background: The implementation of special autonomy of Papua is expected to improve quality of health service at District of Kepulauan Yapen. The reality shows that autonomy that has last for 10 years cannot yet be implemented effectively and there is gap. This is due to the fact that local government policy makers (stakeholders) have not priority on the use of special autonomy budget for immunization program at District of Kepulauan Yapen. Objective: To identify perception of stakeholders aboout the utilization of special autonomy budget of Papua for immunization program at District of Kepulauan Yapen. Method: This was a case study that used qualitative method. Informant consisted of the regent, local government secretary, head of local parliament, head of commission 3 of local parliament, head of local planning council, head of health office, head of Health Problem Management section, head of disease control section, head of health center, immunization staff. Secondary data were obtained from documents of plans and decisions of Kepulauan Yapen District Health Office. Result: Perception of stakeholders about the utilization of special autonomy budget for immunization program was extremely low. Decision making and implementation were made absolutely by Kepulauan Yapen District Health Office. Monitoring and evaluation of immunization program were carried out at minimum frequency and irrelevant with technical guideline because it was related with the local revenue and expenditure budget (APBD) that was always decided far behind the time scheduled by the local government every year. Head of health office still considered immunization as a non priority program. Conclusion: Kepulauan Yapen District Government needed to make regulation on the allocation of special autonomy budget for health office.
