Summary:Background: Public health practice emphasizes the importance of disease management at the individual level to avoid a bigger public health problems. Staying away from health facilities, lack of family support, and social insecurity remain important community social characteristics around Prambanan Subdistrict. Despite of being one of health center's major six program, this program gains lack of attention both from health centers and local governments.This study sought to document best practices and obstacles in the implementation of home care by government health workers. Objectives: This study wants to learn about the efficiency and effectiveness of community based home care in Prambanan subdistrict of Sleman. It specifically examines whether these programs have used the principle of service integration and networks that are important in public health programs in poorly resource countries. Method: This study uses a case study design. We conducted in depth intervew and 1 focused group discussion to care are care providers, health center program unit coordinators, district health officers, and users of home care, private practice midwives, and voluntary health workers in the village of Sumberharjo. The study reports home care visits in 2010. Results: This study shows home care practices from different units of health center: nutrition, maternal and child health, mental health, and control of infectious diseases. The cases analyzed for this study shows that home care was very helpful for poor mothers from counseling to mentoring to make mothers aware and willing to use professionally trained workers and public health facility. Although team work and cooperation between programs has been encouraged, implementation of home care is still organized through vertical program arrangement. Home care will be more efficient and effective if it is based on management program that enable resource planning that consider needs assessment, service delivery integration, and case monitoring of care continuity. Conclusion: Home care is organized individually by separate vertical programs existing in health center. Although the scope of beneficieries is still limited, home care provides a big advantage for the cases so far handled. To make a better efficiency and effectiveness, the study recommends that home care should be organized on the basis of outpatient clinics and group home visiting so that case reduce operational cost.
