(Biomedical Engineering)) The major concern for long-term results of total shoulder arthroplasty is the loosening of glenoid component due to Rocking Horse Phenomena (RHP). Thus, the aim of this study is to analyse the effect of artificial glenoid labrum in minimizing the RHP. Several finite element (FE) analyses were conducted to analyse the contributions of glenoid implant fixation designs affecting RHP. A 750 N load was applied to three different locations (centre, superior-anterior, superior-posterior). Three dimensional (3D) scapula bone was reconstructed which composed of cortical and cancellous bones where the latter was assigned with orthotropic properties. Parameters such as stress at the components and micromotion at the interfaces which had been associated with RHP were analysed. However, the results obtained shows that, stress at the implant, micromotion at the interfaces, and stress critical volume (SCV) remained high, thus indicating the potential of glenoid component loosening. Therefore, glenoid implant with new artificial glenoid labrum was developed in this study to act as an absorber to the glenoid implant. Freeze-thaw (FT) method was used in developing the artificial glenoid labrum using polyvinyl alcohol (PVA) hydrogel. The results showed, 20% PVA and 10% PVA with three FT cycles represent higher (0.4 MPa) and lower (0.1 MPa) compressive modulus of intact glenoid labrum, respectively. In addition, the crystallinity of the hydrogel decreased from the high-PVA to the low-PVA region. Conversely, the visibility of pores increased as it move from high-PVA to low-PVA region. Compared to the standard implant, glenolabrum implant has reduces 51% of stress at the implant and another reduction of 17% was observed for micromotion at the interfaces.
