Published 2014-02-01“…Aunque la ventilación pulmonar protectora (VP) (volumen tidal 6 mL/Kg, presión plateau <30 cmH2O) es una recomendación aceptada en las principales guías clínicas para el tratamiento del Síndrome de Distrés Respiratorio del Adulto (SDRA), hasta ahora los resultados de esta estrategia de ventilación en cirugía se habían estudiado desde el punto de vista de desenlaces subrogados de función pulmonar o mediadores de la inflamación. En New England Journal of Medicine se ha publicado el estudio IMPROVE (Intraoperative Protective Ventilation) en el que se demuestra mejoría de las complicaciones mayores pulmonares y extrapulmonares con la aplicación de la estrategia protectora frente a volúmenes tidal (Vtd) altos, sin aplicación de PEEP (Presión Positiva al Final de la Espiración) ni realización de maniobras de reclutamiento durante la cirugía abdominal.…”
