    “…Results: Implementation coding of emergency patients in Panti Rapih Yogyakarta hospital is carried out by the officers with the assist of another officers in a computerized that using electronic medical records (EMR) who have ICD-10 databases, the other facilities are used as a tool for coding: ICD-10 volumes 1-3, update ICD -10 in 2006, Indonesian - English dictionary, medical dictionary, list of abbreviations that have been standardized in Panti Rapih Hospital in Yogyakarta, and a list of international abbreviations, coding procedures and policies is contained in SPO Number RSPR/S5P2/SPO.24 about coding and indexing, SPO about coding on EMR isnâ��t exist yet because in the make and revision process, the use of EMR provides many benefits in coding process, coding deficiencies in the implementation of EMR is there isnâ��t any instruction notice if there is incomplete fill in item patient data and in terms of human resources, the percentage of accuracy of diagnosis codes of emergency patients on EMR based on ICD-10 by 53.63% and 0% external causes, causes of inaccuracies diagnosis and external cause codes is EMR about coding, coding audits have not been done, and human resources, the efforts that have been made to reduce inaccuracies code are doctors and nurses have to input medical data of patients in EMR in accordance with medical data of patients that are written on sheet of emergency patients on manual medical records, coders ask the doctor to complete the EMR if there are incomplete fill of EMR and diagnosis that be found, coder repair code if there are code that not specific / not in accordance with the ICD-10, coders carry out the coding manually and add the code into the database of ICD-10 on the RME.…”
